  • Receive appropriate, timely, and coordinated services that meet your individual needs.
  • Refuse treatment, services, or caregivers (within legal boundaries).
  • Make advance directives or designate a medical decision-maker.
  • File complaints or express grievances without retaliation or fear of losing services.
  • Confidentiality of your protected health information (HIPAA).
  • Receive a written explanation of your rights in a language and format you understand.
